Upon discharge from a hospital, rehab or care facility, many patients need additional support. At the time of discharge, the facility discharge coordinator is responsible for ensuring their patient goes home to a house that is safe and clean.  Unfortunately, this requirement can be challenging for patients who do not have family members nearby and may need assistance getting their home in order for their return.

Always Best Care Senior Services can fill this need, with the following services to ease the transition from facility to home. Our services include:

  • Transport the patient from the facility to the home
  • Accompany the patient to the home and helping the patient settle back in
  • Clean out the refrigerator and restock it with food (as specified and paid for by the patient)
  • Restock health and beauty necessities such as toilet paper, Depends and other items suggested by the discharge coordinator (at the patient’s expense)
  • Fill prescriptions and pick up new medication at the pharmacy
  • Light housekeeping and trash removal
  • Light laundry
  • Purchase safety items such as non-slip mats, shower chairs and grab (patient is responsible for payment; installation not included)
  • Oversight by our Director of Nursing

WELCOME HOME offers two programs:

Our standard service provides three hours/day of Home Health Aide (HHA), Certified Nursing Assistant (CAN), or companion care services (minimum three days).

Our welcome home plus service offers 24 hour live-in care for the first three days (minimum) following discharge.
